چکیده

INTRODUCTION Iron overload is a common occurrence in children with thalassaemia major and sickle cell disease (due to frequent blood transfusions) or hereditary hemochromatosis. In this patient group, a non-invasive means to quantify iron is extremely attractive compared to conventional liver biopsy for determining total body iron. Recent advances in MRI have enabled non-invasive iron quantification [1,2] and a FDA-approved T2-based technique (FerriScan) is currently available as a surrogate to liver biopsy. However, more validation data is needed on children, and a very rapid T2*-based acquisition would be even more valuable as sedation could be avoided. In this study, we assess the accuracy of a T2*-based approach to quantify liver iron overload in children and compare a widely used constant offset model against other models for analyzing T2* data.

Assessment of Organ Specific Iron Overload in Transfusion-dependent Thalassemia by Magnetic Resonance Imaging Techniques

The consequence of repeated blood transfusions in thalassemia is iron overload in different organs. Magnetic resonance imaging (MRI) is a reliable, non-invasive and accurate method for iron detection in various tissues, hence the introduction of MRI has revolutionized the management of these patients and improved the life expectancy of them. Cardiac MRI T2* has a profound effect not only on est...

Efficacy of Deferasirox (Exjade®) versus Osveral® in Treatment of Iron Overload in Patients with Beta- thalassemia Major in Iran; A Non-randomized Controlled Trial

Background: Iron chelators are an important part of management of patients with thalassemia. It is prudent to compare efficacy of different iron chelators in treatment of iron overload in these patients who receive regular blood transfusion. We aimed to compare the efficacy and safety of available oral iron chelator; Deferasirox (Exjade®) with Deferasirox (Osveral®) in reducing iron ove...

Assessment of Pancreatic Iron Overload in Transfusion Dependent Thalassemic Patients

Advances in the management of transfusion dependent thalassemic patients have improved the survival of these patients. The most important consequence of repeated and frequent transfusions is iron accumulation in vital organs. The magnetic resonance imaging (MRI) is a non-invasive and valid technique for the estimation of iron stores. Despite multiple studies about cardiac and liver MRI T2*, the...

The Effect of Combination Iron Chelating Agents on Reducing the Severity Grading of Heart and Liver Iron Overload in β-Thalassemia Patients

Background: Deferasirox (DFX), Deferoxamine (DFO), and Deferiprone (DFP) are iron chelators that can be used in thalassemic patients with iron overload. Materials and Methods: This clinical trial was performed on 108 thalassemic patients who were randomly divided into group A (n=54) and B (n=54). Group A received combination of DFX and DFP, and group B received DFO and DFP for six months. Seru...

Association between serum ferritin level, cardiac and hepatic T2-star MRI in patients with major β-thalassemia

Background Frequent blood transfusion is often associated with iron overload. Proper use of iron chelators to treat iron overload requires an accurate measurement of iron levels. Magnetic resonance T2-star (T2* MRI) can measure iron level in the heart and liver. Our goal was to see whether an association exists between serum ferritin level and T2* MRI in patients with major beta thalassemia. ...

Correlation between Heart and Liver Iron Levels Measured by MRI T2* and Serum Ferritin in Patients with β-thalassemia Major

Background Transfusion-induced iron overload leads to many complications in patients with β-thalassemia major. This study aimed to compare the level of iron stored in the liver and heart measured by MRI T2 * with ferritin levels in these patients. Materials and Methods This study was done on 52 patients with β-thalassemia major aged 7 to 29 years. Serum ferritin level was checked and heart and ...
